Identifying Needs, Driving Action: Our 2024 Community Health Needs Assessment

At Monadnock Community Hospital, our mission begins with listening—because to serve our community well, we must first understand it. In 2024, we conducted a comprehensive Community Health Needs Assessment (CHNA) to identify the greatest barriers to health in the Monadnock Region.

Through partnerships with local organizations and conversations with community members, we identified five key priorities:

These findings are more than data points—they’re a roadmap for how we invest our time, energy, and resources. The CHNA guides our Community Benefits Program and informs collaborations with area nonprofits, public health agencies, and government organizations to help close gaps in care and support.

With your support and the insights from this assessment, we are tailoring programs to meet the unique needs of our region—whether it’s through expanded behavioral health services, mobile care teams, or financial assistance for families in crisis.
