Patient Portal Access

Welcome to the Monadnock Community Hospital Patient Portal Access Tutorial.

VIDEO TUTORIAL

Healow gives you access to the Monadnock Health Partners (MHP) patient portal and phone App. Where you can message your care team, view your lab results, view your records, request a medication refill, and request an appointment.

You can set up your portal by clinking this link to access the patient login welcome page:

Before you begin contact your MHP provider’s office and ask them to get you web enabled if you have not already done so. You will need to provide them with your email address and mobile phone number.

The App is free and available to download on all Apple and Android devices.

If you have attempted to retrieve your username and password using the login page and still require assistance or if you need immediate assistance during business hours, please contact your Primary Care or Speciality office that set the account up.

For continued technical issues please contact portal.support@mchmail.org or call (603)924-2664 and a representative will get back to you within 24-48 business hours.
