Chronic Care Management
Monadnock Community Hospital is pleased to offer a new program for Medicare patients called Chronic Care Management.
The program was designed by Medicare to help healthcare providers better care for patients managing more than one chronic condition, such as high blood pressure, diabetes, arthritis, cancer, stroke, and obesity. If you have at least two of these conditions, you might be eligible for Chronic Care Management. If you are eligible, we believe you would benefit from the program and encourage you to participate.
If you choose to enroll, we will call you once a month to check in with you on medication management, scheduling appointments, and staying in touch with your healthcare provider. We will also work with you to develop and follow a personalized care plan.
Eligibility
If you are a Medicare subscriber who is currently managing more than one chronic condition, you may be eligible for this program. Chronic Care Management provides a personalized set of activities designed to help keep you as healthy as possible, especially during the COVID-19 outbreak. Taking better care of yourself puts you in an improved position to withstand a COVID-19 infection, and we want to help you do that. More than two-thirds of Medicare-eligible patients have multiple chronic conditions.
Do I qualify?
You qualify for this program if you:
- Have two or more chronic conditions expected to last more than 12 months
- Have visited an MCH medical practice within the last year
How do I receive this benefit?
An MCH representative will answer your questions, help you enroll, and schedule your first call with one of our Care Navigators.
