Understanding care options at home can help individuals and families make informed decisions with confidence. This seminar provides a clear overview of available services and how they support health, independence, and quality of life.

This seminar is presented by Katie Hart, Director of Operations at Home Healthcare, Hospice and Community Services (HCS). In this session, Katie shares practical guidance on how HCS support patients and families at home, including when to consider care and how to access available resources in the Monadnock Region.

This recording is especially useful for family members and caregivers who are beginning to explore support options for a loved one.

Understanding Care at Home

Care at home includes a range of services designed to support recovery, manage chronic conditions, and provide comfort. These services allow individuals to remain in familiar surroundings while receiving professional care. For many families, this approach offers both practical support and peace of mind during times of change.

Home Health and Visiting Nurse Services

Home health services bring skilled clinical care into the home. This may include nursing visits for wound care, medication management, monitoring chronic conditions, and post-surgical recovery. These services are often short-term and focused on helping individuals regain strength and stability after illness or hospitalization.

Hospice and Palliative Care

Hospice care focuses on comfort and quality of life for individuals with serious illness. Care teams provide symptom management, emotional and spiritual support, and guidance for families. Palliative care may also be introduced earlier to help manage symptoms and improve quality of life alongside treatment.

Foot Care and Community Wellness

Foot care clinics provide routine nail care and wellness checks. These services help identify potential health concerns early, particularly for individuals with diabetes or circulation issues, and support overall mobility and independence.

Supporting Families and Caregivers

Care extends beyond the individual to include family members and caregivers. HCS offers social work support, caregiver guidance, and bereavement services. These resources help families navigate decisions and feel supported throughout the care process.

Community-Based Programs

Programs such as Meals on Wheels, transportation services, and Walk-In Wednesdays help individuals remain independent and connected to their community. These services address both medical and social needs, which are equally important for overall well-being.

Getting Help

HCS offers Walk-In Wednesdays every Wednesday from 1 to 3 PM in Keene and Peterborough, with no appointment needed. This provides an opportunity to ask questions, discuss individual needs, and get connected to appropriate services.
