What helps older adults stay independent, active, and supported as they age? In this free seminar from Monadnock Community Hospital, clinical experts explain the Age-Friendly Health Systems “4Ms” framework and discuss mobility, cognitive health, medication safety, and planning for future healthcare decisions.

Healthy aging involves more than managing medical conditions. It also includes maintaining mobility, protecting cognitive health, staying connected to activities that matter, and making informed healthcare decisions over time.

In this Health & Wellness Seminar, experts from Monadnock Community Hospital discuss practical ways older adults and caregivers can support independence, safety, and quality of life through the Age-Friendly Health Systems “4Ms” approach.

About This Seminar

The “4Ms” framework focuses on four key areas that influence health and well-being as we age:

  • What Matters
  • Medication
  • Mentation
  • Mobility

The seminar explains how these areas work together to support safe, patient-centered care and help older adults continue participating in the activities and relationships that matter most to them.

What You’ll Learn

Mobility and Fall Prevention

The seminar explores how vision, balance, hearing, neuropathy, medications, and prior injuries can affect mobility and increase fall risk over time. Presenters also discuss exercise, movement, mobility aids, and physical therapy strategies that can help older adults stay active and independent.

Understanding Cognitive Health

Presenters explain the differences between depression, dementia, and delirium, including warning signs, stages of dementia, and when to seek medical support. The discussion also highlights the importance of staying socially engaged and proactive about cognitive health.

Medication Safety for Older Adults

As people age, medications may affect the body differently. The seminar discusses medication side effects, the Beers Criteria, medication interactions, and why patients should regularly review medications with healthcare providers and pharmacists.

Planning Ahead and “What Matters”

The final section focuses on advance directives, healthcare decision-making, autonomy, and aligning healthcare with personal goals and priorities. Presenters encourage participants to speak openly with loved ones and providers about the care and activities that matter most to them.
