Monadnock Community Hospital Launches New Core4 Weight Management Program

New Program Offers Practical Support for Long-Term Success

Many people are looking for a realistic approach to improving their health but often struggle to find programs that fit their lives.

The new Core4 Weight Management Program was developed to provide participants with the tools, support, and education needed to make meaningful lifestyle changes.

Beginning June 22, the program will bring together individualized nutrition counseling, weekly group sessions, and ongoing accountability in a supportive environment.

Rather than focusing on restrictive diets or short-term solutions, Core4 emphasizes sustainable behavior change.

Participants will work with Registered Dietitians to build practical skills related to nutrition, movement, healthy routines, and overall wellness.

The program centers on four pillars:

  • Eating
  • Exercise
  • Education
  • Energy

Program Includes

  • Two one-on-one Registered Dietitian sessions
  • Weekly group education and support sessions, held Wednesdays from 5:30–6:30 p.m.
  • Personalized goal setting
  • Evidence-based nutrition strategies
  • Additional maintenance support
