All skilled nursing patients stay in their own rooms, and enjoy the privacy of their own bathroom.

You will need a referral. Referral information can be faxed to: 603-924-4686.

Once we receive the referral:

Our treatment team will meet to discuss your needs and our ability to meet them.
If you meet the admission criteria, our assessment nurse will contact your insurance carrier to verify benefits, determine the need for additional information and obtain approval for a transfer to MCH.
Once approved, our nurse will notify the referring hospital's discharge planner and work with our admitting office to coordinate your transfer/admission.
