Meet Our Community Paramedics
Tom Dardas, CCP-C, CP-C Tom has been in emergency medical services (EMS) for over 30 years. He served as a combat and flight medic with the U.S. Army for 14 years deploying to Iraq during the Persian Gulf War. Tom is dual boarded-certified as a critical care paramedic (CCP-C) and community paramedic (CP-C). He completed his community paramedic clinical rotations here at MCH. Tom has been a Community Paramedic at MCH since the program since it started in April 2020.
Joshua Patrick, MPSLA, CP-C Josh has been in emergency medical services (EMS) for over 15 years. He earned a Master of Public Safety Leadership and Administration degree with a concentration in mobile integrated healthcare and is currently a Ph.D. candidate and active researcher. Josh is a board-certified community paramedic (CP-C) and an adjunct instructor of community paramedicine. Josh is currently the Lead Community Paramedic clinician at MCH and has been with the MIH program started in April 2020.

About: Louise is the Program Manager for the Mobile Integrated Healthcare (MIH) program at Monadnock Community Hospital. She has a Bachelor’s degree in Communication and Business, along with 30+ years of experience in the healthcare field. Louise engages with many community initiatives that address critically important social determinants of health. In her free time, she enjoys training for and taking part in triathlons.

About: Tom has been in emergency medical services (EMS) for over 30 years. He served as combat and flight medic with the U.S. Army for 14 years deploying to Iraq during the Persian Gulf War. He is dual board-certified as a critical care (CCP-C) and community paramedic (CP-C). Tom is currently finishing an Associate of Science in Community Paramedicine. In his free time, he is working on obtaining his private pilot’s license.

About: Josh is the lead community paramedic clinician in the Mobile Integrated Healthcare (MIH) program at Monadnock Community Hospital. He holds bachelor’s degrees in emergency and community paramedicine, a master’s degree in MIH program administration, and is currently pursuing a PhD. Josh is an adjunct senior lecturer of community paramedicine and human anatomy and physiology and an active researcher. He is a board-certified community paramedic (CP-C) and an item writer for the IBSC community paramedic board exam. In his free time, he enjoys hiking with his husky, Granite.

Key highlights of the award-winning program include:
- Providing primary care patient services
- Sending community paramedics directly to patients' homes
- Offering services like wound care and vaccinations
According to the New Hampshire Department of Health and Human Services, this program is remarkably effective. In 2022, it represented 49 percent of total visits across the state's 12 Mobile Integrated Healthcare programs, making it one of the busiest in New Hampshire.
Sean Burke, The Sentinel's president and COO, praised the program for "meeting a dire need in our community" by reaching patients where they are and improving both health and social factors.
The award was accepted by Joshua Patrick, lead community paramedic clinician, and Karen Bednarski, dispatcher, highlighting the team's crucial role in addressing healthcare gaps in the Monadnock Region.
