MCH Welcomes Home Healthcare Hospice and Community Services: Expanding Local Care

Monadnock Community Hospital (MCH) is excited to announce that Home Healthcare, Hospice and Community Services (HCS) will be joining our campus, significantly enhancing the continuum of care for our community. This partnership demonstrates our shared commitment to delivering comprehensive, compassionate healthcare services.

About HCS

Founded over a century ago, HCS is a 501(c)(3) not-for-profit organization providing essential health and support services. Serving more than 2,000 individuals daily across 52 communities in southwestern New Hampshire, HCS offers:

  • Visiting Nurses: A dedicated team of nurses, social workers, therapists, and caregivers providing expert, compassionate care in patients’ homes, ensuring personalized support and comfort.
  • Hospice Care: Holistic end-of-life care focusing on enhancing quality of life, offering comprehensive physical, emotional, and spiritual support to patients and their families.
  • Healthy Starts: An innovative initiative investing in children and families to promote health, stability, and resilience, establishing a strong foundation for lifelong well-being.
  • Foot Clinics: Nurses provide essential foot and nail care, massage feet with lotion, assess for foot issues, and make referrals if needed.
  • Nutrition for Seniors: Comprehensive meal services including prepared meals for homebound seniors and congregate nutrition programs, supporting older adults’ health and independence.
  • Transportation Services: Critical assistance helping individuals access medical appointments and daily activities, promoting mobility and independence.

Services HCS will offer at MCH:

Starting in December HCS will open its new office in Suite 208 of the Medical Arts Building on the MCH campus. This strategic relocation from their Peterborough location will foster closer collaboration and more streamlined services.

The new MCH location will primarily offer:

  • Foot Clinics
  • Healthy Starts Program
  • Personalized client and family consultations
