Already certified as an Age-Friendly Health System, MCH has now earned the Committed to Care Excellence designation from the Institute for Healthcare Improvement (IHI).

This recognition validates our demonstrated excellence in implementing the nationally recognized 4Ms framework: What Matters, Medications, Mentation, and Mobility, transforming care outcomes for every older adult we serve. By putting patients’ goals first, reviewing medications carefully, supporting cognitive health, and promoting safe mobility, we ensure our care is personalized and respectful of everyone’s needs.
