(Manchester & Peterborough, NH) – GraniteOne Health is pleased to announce that members Catholic Medical Center (CMC) Physician Practice Associates and Monadnock Community Hospital (MCH) Physicians have been named to Harvard Pilgrim Health Care Physician Group Honor Roll. CMC and MCH are the only New Hampshire organizations on the 16th annual list. The Honor Roll reflects exceptional performance in areas of adult and pediatric acute, chronic, and preventative care. Harvard Pilgrim identifies these physician groups by measuring 15 different metrics against national standards from the Healthcare Effectiveness Data and Information Set (HEDIS®). Examples include appropriate treatment for children with upper respiratory infection, including comprehensive diabetes care, and breast cancer screening. This filing comes after an extensive period of due diligence, approval of the agreement by all three hospital boards, and, with respect to CMC, approval of the Roman Catholic Bishop of Manchester. CMC previously announced a Letter of Intent (LOI) to affiliate with Huggins Hospital in November, 2015 and an LOI with Monadnock Community Hospital in January, 2016. The agreement being reviewed by regulators will result in a more integrated healthcare system to serve the hospitals’ respective communities.

This New Hampshire‐based, New Hampshire‐focused collaboration allows us to effectively use our resources and expertise to improve quality, cost, and access to health care.
