Dr. Fazelat is a vitreoretinal specialist. Dr. Fazelat treats patients suffering from Age-related Macular DegenerationDiabetic Retinopathy, Retinal Artery, and Vein Occlusion, Macular Edema, Epiretinal Membrane, Vitreomacular Traction, Macular Hole, Retinal tears, and Retinal detachment.

Dr. Fazelat completed his Medical Doctorate at Boston University School of Medicine. Dr. Fazelat completed a General Ophthalmology residency at Albert Einstein College of Medicine. Subsequently, Dr. Fazelat completed a 2-year private, retina fellowship affiliated with Massachusetts Eye and Ear Infirmary, Harvard Medical School.

Dr. Fazelat has published in several peer-reviewed articles, been a guest speaker on radio talk shows, and has given presentations on many topics which include “Genetic Markers in Age-related Macular Degeneration”, “Macular Holes”, “Retinal Vein Occlusion”, “Ophthalmic Emergencies” and “Diagnosis and Treatment of Rhegmatogenous Retinal Detachment”.

Dr. Fazelat is a board-certified Ophthalmologist, a diplomate of the American Academy of Ophthalmology, and a member of the American Society of Retina Specialists. He is affiliated with and an active staff member at the Elliot Hospital.

Dr. Fazelat was recently recognized as one of the “Top 40 under 40” for the class of 2014 by the New Hampshire Union Leader. Dr. Fazelat has also been chosen by his peers as one of New Hampshire Magazine’s “Top Doctors” for 2014, 2015, 2016, 2022 and 2023 in the field Ophthalmology.
