Diabetes Education Program

Welcome to Monadnock Community Hospital’s Diabetes Education Program!

The only Diabetes Self-Management Education program accredited by ADCES (Association of Diabetes Care and Education Specialists) in southwestern New Hampshire
The program offers a variety of services, including:

  • Individual and group education sessions on healthy eating, exercise, monitoring, proper use of medications, and problem solving
  • Medical nutrition therapy (MNT)
  • Support groups

The program is open to all patients with diabetes who live in the Monadnock Region of New Hampshire. Services are provided in person or via telehealth.

Our program is dedicated to providing high-quality diabetes education to our patients. We are proud to be accredited by ADCES, the Association of Diabetes Care and Education Specialists.

At Monadnock Community Hospital, we understand the importance of equipping patients with the knowledge and skills necessary to effectively manage their diabetes and minimize potential complications. Through our program, we focus on teaching essential self-management techniques that empower individuals to take control of their health.

Our comprehensive curriculum covers various aspects of diabetes management. We emphasize the importance of healthy eating habits, regular exercise, monitoring blood glucose levels, understanding and properly using medications, as well as problem-solving strategies for any challenges that may arise.

Meet Our Team

Kaitlyn McCarthy, Registered Dietitian Nutritionist/Certified Diabetes Educator

Kaitlyn McCarthy

Kaitlyn, clinical nutrition manager at Monadnock Community Hospital, is a member of the certified diabetes team. She received her Bachelor’s degree in Nutritional Science from the University of New Hampshire. She then went on to complete her dietetic internship at the University of Saint Joseph in West Hartford, Connecticut. After graduating, she found her niche in clinical nutrition, helping people improving their health and wellness through food. Having several close family members with diabetes is what sparked her interest to get her CDCES and become a diabetes educator. When she is not working, she can be found spending time with friends and family, cooking and baking, and spending time outdoors hiking, swimming, skiing, or kayaking.

Monthly Diabetes Support Group

Join us every 3rd Thursday of the month for our Diabetes Support Group. This is a welcoming space where individuals living with diabetes can connect, share experiences, and learn from each other. Whether you are newly diagnosed or have been managing diabetes for years, you’ll find encouragement, practical tips, and a sense of community.
This group is facilitated by: Whitney Hightower, MS, RDN, LD.
