Dates: Thursday, October 23 & Thursday, October 30 Time: 8:00 AM – 4:00 PM Location: Jaffrey Family Medicine
Our trained staff will guide you through every step of completing your Advance Directive, a legal document that explains your wishes for medical care if you are ever unable to speak for yourself. This includes:
Durable Power of Attorney for Healthcare (DPOAH): Naming someone you trust to make medical decisions on your behalf.
Living Will: Outlining the kinds of life-sustaining treatments you do or do not want.
Free forms, resources, and on-site notary services will be available. Take control of your future health care in a supportive, one-stop setting.
