The Art of Aging Gracefully: Living Well as We Age

April 16 @ 5:30 pm - 6:30 pm
A smiling nurse in blue scrubs sits closely beside an older adult with short gray hair. Both are laughing together in a bright clinical setting, representing supportive care, connection, and healthy aging.

Aging brings new opportunities to focus on health, independence, and overall quality of life. Join Monadnock Community Hospital for an informative seminar, The Art of Aging Gracefully: Living Well as We Age, designed to support well-being at every stage of life.

This panel discussion will introduce the Age-Friendly Health Systems approach, centered around four essential areas of care known as the 4Ms: What Matters, Medication, Mentation, and Mobility. Together, these principles help ensure that care for older adults aligns with personal goals, promotes safety, and enhances quality of life.

Experts from Monadnock Community Hospital will share practical insights into how these areas influence healthcare decisions and how individuals and families can advocate for care that truly reflects their priorities.

Topics will include:

  • Maintaining mobility and strength to support independence
  • Recognizing and addressing memory and mental health changes
  • Understanding medications and their impact on aging
  • Ensuring healthcare decisions align with personal goals and preferences

The presentation will conclude with an interactive Q&A session with the panel, followed by access to additional resources and information.
