Many families associate home healthcare or hospice services with a medical crisis or the final stages of illness. In reality, these services support individuals and families at many points along the care continuum and can improve comfort, independence, and quality of life long before a crisis occurs.

Join Home Healthcare, Hospice & Community Services (HCS) for a free educational seminar designed to help older adults, caregivers, and adult children supporting parents understand available services and how care is coordinated across home, community, and hospital settings.

This session will provide a clear, practical overview of the services available to support people at home and in the community and explain how these services work together with hospital and outpatient care.

What You’ll Learn:

  • How home healthcare and visiting nurse services support safety and independence at home
    • When additional support may be helpful and how hospice or palliative care fits into long-term planning
    • What community-based services, such as foot care clinics and family support programs, offer
    • How care is coordinated across home, community, and hospital settings
