Heart Smart

Heart-Smart is a Chronic Disease Management Education and Exercise Program for those individuals who do not qualify for Cardiac Rehabilitation and exhibit specific functional limitations to justify physical therapy treatment.

A referral from your Primary Care Physician is required. This program is covered by most insurances.

Education plays an important role in congestive heart failure management. Topics include:

Our 20 session outpatient program is designed to treat a variety of diagnoses. The exercise, which incorporates aerobic training, stretching, and strength training, is individually tailored to each patient's abilities.

Referring a Patient

If you have a patient you feel is a good candidate for Heart Smart and would like to refer him or her to our program, you can fax it to us at 603-924-4651. We will be happy to then contact the patient and discuss our program with him or her.

Required information to be sent with the referral: Most recent H&P, EKG, ETT (if there is a remote history of CAD or state the patient does not need one based on your clinical judgment), lipid profile, A1C, reports of any cardiac testing (ECHO, CATH, etc).


Prior to beginning the program, each patient will be scheduled for an initial evaluation with the registered nurse. During this time we will perform a physical assessment and discuss risk factor management specific to the individual’s needs. Clients with CHF are also enrolled in a five part lecture series.

Session Monitoring

Upon arriving to class resting vital signs and weights are documented. Heart rates, oxygen saturation and blood pressures are monitored throughout exercise. The exercise prescription is individualized and is progressed over time. If the patient has diabetes, blood sugar will be checked prior to and following exercise.
